请假条包括病假条、事假条、续假条等。写请假条主要把请假原因和请假时间写清楚，请假的理由要讲充分。同时也要注意内容要简单明了，开门见山说明情况，不必赘述。如果有证明请假原因的证据，如医生证明等要作为附件上交。

写请假条应注意以下几点：

1.要注意写清条子的名称。在第一行的正中间要写上“请假条”三个字。

2.要写明是向谁请假。在第二行或第三行，顶格写明批准请假人的职务或姓名。

3.要写请假的原因和期限。

4.最后要写上请假人的姓名和写请假条的日期。
